Exposure Reports

Personal Exposure Report Forms are archived for Local 1171 members by Sister Sandy French. Copies are available from two sources: 1) The Local's website (see below) and the Santa Clara Fire Department's network “I” drive.

To use the form on this page, click on the Word document link below. After filling out the form, print a hard copy for your personal file. Save a copy on your computer and email a copy to Sandy French at sfrench@ci.santa-clara.ca.us Sister French will archive your electronic copy and store all information.
